Explicit CLAP discovery and trusted loading

Scoped release/choke directions

with_scoped_note_input(true) and with_scoped_note_output(true) require explicit CLAP ports for their corresponding directions. Input converts NoteOffScoped and NoteChoke without loss of optional channel/key/native ID, independently of identified On/Off and expression flags. Complete disabled/invalid input rejects pre-native. Output preserves wildcard-channel/key Off and choke; concrete native Off retains legacy/identified selection. Default scoped output filters.

Exact selected-port raw scope checks reject channel/key/ID below -1 and out-of- range concrete channel/key; other or wildcard ports filter. Off velocity narrows checked finite native 0..1 to f32; choke's ignored native velocity is not validated or carried as a scalar. Prepared capacities, input zero-frame rejection, output fault/nonrollback and retirement rules remain unchanged. Voice release/termination and NoteEnd completion belong to distinct receiver/protocol policies. Linked f32/f64 fixtures test 1000 blocks per selection, exact echo/audio timing, invalid prefix rejection, capacity/malformed output history and main-thread reclamation without measured processing heap actions. See decision 0136.
